Aged care at home

Stronger, steadier, and still at home

Strength, balance and confidence work for older adults — in the hallway, on the back step, at the kitchen bench. Funded through Support at Home and Home Care Packages.

The home is the gym

For an older adult, the exercises that matter most are the ones attached to daily life: standing from that armchair, managing those three back steps, carrying tea from the kettle to the table. We treat in the exact spots where confidence has been lost — which is why home-based therapy works so well for this age group.

Sessions are unhurried. There's time to warm up, practise properly, rest, and talk through what's changing. Family members are welcome to sit in, and if they can't, they get a plain-English progress update every quarter and can ring any time in between.

Falls: prevented, not just treated

One in three Australians over 65 falls each year, and a first fall often starts a spiral of fear and inactivity. Structured strength and balance training is the best-evidenced way to break that cycle. Every aged-care client gets a falls-risk assessment, a progressive program, and practical home-hazard advice — the grab rail conversation happens with the actual rail, the actual step, the actual bathroom.

Jimmy's clinical background is exactly this work: years in Western Sydney hospitals across acute stroke, rehabilitation and geriatric care, plus APA Gerontology Physiotherapy Level 1 and Dementia Essentials training. It's the foundation the whole service is built on.

Comfortable with complexity

Arthritis on top of a heart condition on top of early memory loss — that's normal, not exceptional, for our clients. We're experienced with dementia care, post-hospital reconditioning, Parkinson's, stroke recovery and palliative support, and we coordinate with GPs, geriatricians and care managers so nothing falls between the cracks.

Care team connected

We report to care managers and GPs without being chased — clean notes, on time.

Funding: Physiotherapy is classified as clinical care under Support at Home (the program that replaced Home Care Packages in November 2025) — meaning it's fully government-funded within your budget, with no participant contribution. Existing HCP clients were moved across automatically with a no-worse-off guarantee. We invoice your provider directly; you just book the visits. More on fees & funding →

Good questions

Asked all the time

What happened to Home Care Packages?
Support at Home replaced Home Care Packages on 1 November 2025. If Mum or Dad had a package, they were moved across automatically — no reassessment, and a no-worse-off guarantee on costs. For physio, the news is genuinely good: it's classified as clinical care, which carries no participant contribution.
Will physio use up the whole budget?
Support at Home budgets are quarterly, and clinical care like physiotherapy is fully funded within them — no co-payment. We'll talk frequency honestly: most clients do best with a focused block of weekly visits, then a maintenance rhythm that's much lighter on the budget.
Mum has dementia — is home physio still realistic?
Often it works better at home than anywhere else. Familiar surroundings, one consistent physiotherapist, routines built into the day she already knows. Our team trains specifically in dementia care (Dementia Essentials, Dementia Australia), and we adjust pace and communication to the person in front of us.
Can you start before the package or budget is approved?
Yes. Many families start privately during the wait and switch the funding over once it's approved — the changeover is one phone call on our side. Don't let the paperwork delay the recovery.
