Your information, handled with care
The plain-English version first: we collect only what we need to care for you, we store it securely, and we never sell it. The detail follows.
Who we are
JX Physio (ABN [00 000 000 000]) provides mobile physiotherapy services across Sydney. We are bound by the Privacy Act 1988 (Cth), the Australian Privacy Principles, and — because we handle health information — the additional protections that apply to health service providers in NSW.
What we collect
- Contact details — name, phone, email, address — from you or the person referring on your behalf.
- Health information — relevant history, clinical notes, assessments and progress records created during care.
- Funding details — such as NDIS numbers, plan management contacts, or aged-care provider details, used only to arrange billing.
- Website forms — what you type into our referral, quiz, contact and careers forms is transmitted securely to our practice management system and used only to respond to you.
How we use it
To provide and coordinate your care, communicate with people you’ve asked us to keep informed (family, support coordinators, care managers, GPs), arrange funding and invoicing, and meet our professional record-keeping obligations. We do not use your information for marketing without your consent, and we never sell it.
Who we share it with
Only the people involved in your care and funding — and only what they need: your nominated family contacts, your care team (GP, specialists, other therapists), your plan manager or aged-care provider for invoicing, and government schemes where required for claims. Beyond that, we disclose information only when the law requires it or to prevent a serious threat to someone’s safety.
How it’s stored
Clinical records are kept in our secure practice management system, hosted in Australia, with access limited to the clinicians and staff involved in your care. Records are retained for the periods required by law for health records, then securely destroyed.
Your rights
You can request access to your records, ask us to correct them, or make a privacy complaint by contacting info@jxphysio.com.au or 0449 918 993. If you’re not satisfied with our response, you can contact the Office of the Australian Information Commissioner (oaic.gov.au).
This website
Our site doesn’t use advertising trackers. Standard server logs (pages visited, approximate location, device type) are collected by our hosting provider for security and performance. Form submissions travel over an encrypted connection.
